Privacy Policy

1. Introduction

This Privacy Policy explains how I collect, use, and protect your personal health information when you use my nutrition services, including online booking, virtual appointments, and other communications. I follow Ontario’s Personal Health Information Protection Act, 2004 (PHIPA) and the standards of the College of Dietitians of Ontario.

2. What Information I Collect

Your name, email, phone number, and address
Relevant health information that you provide (for example: medical history, medications, lab results, nutrition concerns, and lifestyle information)
Appointment history and payment information

I only collect information that is reasonably necessary to provide safe, effective nutrition care and to run my practice.

3. How Your Information Is Used

Provide safe, evidence-based nutrition care
Assess your needs and create or update your care plan
Schedule and manage appointments
Process payments and manage billing
Communicate with you about your care, appointments, and follow-up
Maintain records as required by law and by the College of Dietitians of Ontario

I do not sell your personal health information.

4. How Your Information Is Stored and Protected

Your records are stored electronically in secure systems (for example, Square and encrypted file storage). Some services may store data on servers outside of Canada (for example, in the United States). When information is stored outside Canada, it may be subject to the laws of that country. I take reasonable steps to protect your information against loss, theft, and unauthorized access, including:

Using secure, password-protected systems and devices
Limiting access to those who need the information to do their work
Following policies for secure handling, storage, and disposal of records

5. Retention of Records

In line with Ontario regulations and professional standards: Records are kept for at least 10 years after your last visit, and for minors, records are kept for at least 10 years after your 18th birthday. After this time, records are securely destroyed or anonymized.

6. Access to Your Records and Your Rights

Request access to your records and receive a copy
Ask for corrections if you believe information is inaccurate or incomplete
Withdraw consent for certain uses or disclosures, except where disclosure is required or permitted by law

7. Sharing of Information

I do not share your personal health information with third parties without your consent, except in situations where I am required or permitted to do so by law. This may include:

Situations where there is a serious risk of harm to you or others
Court orders or other legal processes
Mandatory reporting to public health or regulatory authorities

8. Virtual Care and Electronic Communication

Virtual appointments are provided by secure video or phone. I take reasonable steps to protect your privacy when using electronic tools. All electronic communication carries some risk of interception or technical failure. By booking, you consent to receiving care through these methods and understand the risks.
